We have a fantastic opportunity in a newly created role for an experienced Registered Nurse, Quality and Education Manager or Care Manager to step into the people leadership space as Community Nurse Team Lead. In this newly created role, you will lead and manage the growing clinical care team of registered nurses and enrolled nurses (currently 16) to achieve optimal client and clinical care outcomes. 

About the Role:

Provision of quality Clinical Nursing services to our valued aged care clients in their own homes with key responsibilities being:

  • Providing clinical leadership, coaching and the delivery of high-quality nursing clinical care; ensuring the team meets organizational goals and standards (50% of the role)
  • Ensuring delivery of evidence-based practice and risk management by adhering to best practice policy and procedures (30% of the role)
  • Providing Clinical Service delivery within Model of Care, improving client care outcomes and managing the triage of referrals
  • Carrying a small clinical caseload (20% of the role)

 

Day to day this looks like:

  • Supporting the development and implementation of clinical competencies and oversight to ensure appropriate service delivery in the nursing service
  • Identifying and leading service development and quality improvement activities to advance clinical nursing care
  • Supporting the professional growth and development of team members through mentoring, coaching and training to develop clinical competencies
  • Leading clinical forums, providing updates sharing of clinical discussion / education
  • Conducting regular audits and reviews of clinical practices to ensure quality standards are met
  • Implementing and monitoring quality improvement initiatives and client safety protocols
  • Maintaining a clinical caseload and providing expert clinical care assessments and interventions to achieve client outcomes
  • Supporting team members to meet productivity targets and implementing strategies to address shortfalls
  • Monitoring and evaluating client progress, adjusting treatment plans as necessary
  • Managing workload and triage of referrals – for self and broader clinical care team
  • Participating in meetings as clinical representative and involvement in Quality Improvement activities as required for Clinical governance, audits, practice quality and risk

To be successful in this role you’ll need

  • Allied health professional registration – Registered Nurse - with current AHPRA 
  • At least 4 years' experience in community practice or residential aged care 
  • Strong organisation and management skills
  • Demonstrated knowledge of Aged Care Standards
  • Demonstrated understanding of clinical care – assessment and intervention
  • Demonstrated ability to work in a team setting autonomously
  • Current drivers licence and ability to travel as required
