  • Location – Primarily work from home role with office days required on occasion for training or meetings

We’re thrilled to announce that our Aged Care Assessment Services (‘ACAS’) team is expanding, creating exciting opportunities! ACAS positions offer flexibility and autonomy and are based across both metropolitan and regional locations, with most roles involving local travel within each region.   We have an exciting opportunity for a Clinical Triage Delegate to join the team.  

What will your role look like? 

Your role will assist older people in navigating the assessment process for aged care services by conducting thorough triage evaluations to identify the appropriate assessment pathway and prioritize their needs. 

Day to day this includes:  

  • Establishing rapport to create a safe and comfortable environment for the client to express their concerns and to confirm screening information utilising the Integrated Assessment Tool (IAT) Tool  
  • Determining eligibility and assessment priority and pathway  
  • Conducting risk assessments based on client vulnerability   
  • Gathering information relevant to client circumstances and confirming the information within the referral

  • Maintaining currency of knowledge relating to the Aged Care Act and relevant Aged Care frameworks and standards  

To be successful in this role you’ll need: 

  • Excellent Communication and Customer Service skills to both internal and external stakeholders 
  • Excellent planning, organising and problem-solving skills   
  • Effective negotiation, advocacy and conflict resolution skills   

  • Tertiary qualifications – in a health-related discipline directly related to health, aged care or related specialist area. E.g. registered nurses, medical officers, occupational therapists, physiotherapists and social workers  

  • Current unrestricted registration with the Australian Health Practitioners Regulation Agency (AHPRA) or part of or eligible to be part of a relevant professional association.  

  • Completion of training requirements as specified under the My Aged Care Workforce Learning Strategy 2023.   

  • At least one years’ aged care assessment experience preferred  

  • Willingness to apply for a Police Check
